Location: Western Pennsylvania (Pittsburgh area)

Type: Full-Time |Pay-per-visit

Schedule: Monday–Friday | Limited weekends

Compensation: Base + $15K annual bonus paid out quarterly + mileage reimbursement

Position Summary

The Registered Nurse (Home Health) provides comprehensive patient care in a home setting, coordinating with physicians and interdisciplinary teams to ensure high-quality outcomes and continuity of care. This position supports patients throughout Western Pennsylvania and offers flexibility, autonomy, and a structured onboarding program.

Key Responsibilities

• Deliver skilled nursing care to patients in their homes in accordance with the plan of care.

• Perform initial and ongoing assessments of patient needs, condition, and environment.

• Administer medications, treatments, and wound care per physician orders.

• Educate patients and family members on disease management, medications, and care plans.

• Collaborate with physicians, therapists, and other clinicians to optimize patient outcomes.

• Document all care activities accurately and promptly using electronic medical records.

• Maintain compliance with federal, state, and organizational regulations.

Required Qualifications

• Active Registered Nurse (RN) license in Pennsylvania

• Associate degree or higher in Nursing

• Minimum 1 year of RN experience (Home Health preferred)

• AHA BLS required; AHA ACLS accepted

• Valid driver’s license, reliable insured vehicle, and ability to travel within assigned territory

• Strong communication, organization, and time-management skills

Preferred Qualifications

 

• Prior Home Health or Hospice experience

• Proficiency with OASIS documentation

• Experience in wound care, IV therapy, or post-surgical management
